Hello! My client wants to add a field on the first slide where the user/learner enters his/her name. After completing the course and the quiz, the name and score will be emailed to an admin.
Based on what I found here and here, it's a Storyline feature. Is this possible in Presenter '09?
I can only point you in the direction that I think you are heading (since some of this is not supported by Articulate). This article will help you understand and analyze the e-mailed data from Quizmaker. Here is an article regarding adding student's name. Hope this helps!
I looked into the links you provided. Based on this, I'm supposed to look for quiz.js in the quiz_content folder; I do not have such a file. I checked 2 other published projects and I still do not have a quiz.js file.
Also, when I click the send email button, I get a blank page. I checked this page and everything is spot on on my end. I am testing this locally because it will not be uploaded to an LMS.
The method provided above will only work for stand-alone Quizmaker quizzes, and not for Quizmaker quizzes that are embedded within an Articulate Presenter course.
Unfortunately, there is no support for the functionality you are seeking within Presenter, but it would make a great Feature Request!

Thanks for the link but it does not explain why I am unable to locate the quiz.js file as discussed here.
