It sounds like you may need to download Flash Player for Internet Explorer (ActiveX) for the preview to work (there are separate versions of FP for IE and Firefox).
I have had a similar experience... I am not sure why it does it; however, I have had more luck converting the FLV to an MP4 and importing it. I use Format Factory to convert the files. When I convert them I use the AVC(H264) Video Encode option (See picture below).
