LMS reporting failed despite SCORM set to passed/incomplete

Has anyone seen this before? Our LMS (Moodle) is reporting some people as 'failed' despite the reporting set as 'passed/incomplete'. There are also people with statuses of passed and incomplete. The SCORM was set to track based on the number of slides and so none of the quizzes should be sending back as failed. In fact we are finding it hard to replicate a 'failed' attempt, even when all of the quizzes are answered incorrectly. Any ideas? Thanks

Sorry to hear of your troubles and may I ask if you have also tested your course in the SCORM Cloud to see if you find the same behavior there? my guess is that you have a masteryscore set. in Moodle 3.1 we added a new setting that lets you override this and operate a bit more like the SCORM Cloud default behaviour called "masteryoverride" - if you are using an older version of Moodle the only option is to manually remove the masteryscore node from the imsmanifest.xml file inside your SCORM package.

SCORM Cloud used to operate the same as Moodle's default and they changed the default behaviour a while ago and ran into a few issues as many scorm packages expect this behaviour:
