I had a little trouble finding information for you on this. However, I did find a screencast that may get you on the right track. It may not be exactly what you're looking for, but hopefully it will help with what you're trying to accomplish.
Take a look:
You may also want to take a look at the original discussion for this topic (fill-able form in Presenter), here.
The screencast is really helpful, but unfortunately it doesn't serve the purpose. I want the user's name to be displayed throughout the course and later it could be used for correspondence.
Sorry about that - I didn't realize you would need it to display through the course. Storyline does provide this type of feature, with variables, if you're interested in looking at it. I'm not sure if you'd be able to do the same type of thing in Presenter, though. You may want to post this question over in the Building Better Courses section of the forums for a little more exposure to this topic. Maybe someone will be able to provide some advice on creating this type of interaction in Presenter.
Good luck with the project, Sampurna! I'm curious to see what you end up using for this. If you find something, please share
