assigning grade/points to likert questions - want to tally score

I'm attempting to set up a survey-type assessment where the person's manager will use the tool to rate the employee on how s/he delivered a presentation. Rating scale is 1-5, 1 being poor and 5 being outstanding, with total number of questions at 9 and total points possible at 45, with a passing score being 35 or greater out of 45. I've tried a number of different graded question/survey question formats in Quizmaker 13 and can't get the rating on each question to come out as a total at the end -- so is what I'm trying to do even possible in Quizmaker? If so, guidance is appreciated. Then, how do I change the likert scale description (default is strongly agree to strongly disagree)? Thanks for your help.

You can modify the scale itself to display custom scale "levels", similar to those you mentioned,
You can modify the scale labels in "Form View" by clicking on "Scale.." at the bottom:
