As far as I know, unless you use a hyperlink to a site to show an image (which may not be very appealing to learners to view outside of the course), you cannot add an image to the Hotspot feedback. However, I did find a tutorial on creating custom feedback that may work for you. You can find it here. I know it's not exactly what you're looking for, but with this you'll be able to display additional images on the blank slides mentioned in the tutorial.
