Yes, I can open each the question, but it defaults to slide view. I have lost the part on the right that allowed me to toggle between form view and slide view, and I a) can't get that window back, and b) do not know how to access form view any other way.
Please advise.

Thanks for the screenshot - now I understand! You'll need to hover your mouse over to the right hand side and you'll see a grey line appear that you'll click on and drag back to the left. It's very similar to this screenr I've recorded on how to find it within Storyline.

Are you using SL1 or SL2? In SL2 you have the option to undock some of the panels such as that for triggers and states on the bottom. You can re-dock them all in as described here.

Studio '13 and Storyline are different products and therefore have different options - but since you posted in a Storyline forum, I shared directions based on that.
For Quizmaker '13 there is a little carrot or arrow at the top right hand side that you can toggle to hide the form view/slide view options. So if you're only missing that element - you'll want to look for the carrot. If everything is missing on the right hand side, then you'll need to use the "drag" back to the left as mentioned above. The latter can be tricky to see at first, but you'll see a greyish bar appear on the right - that's what you want to click to drag.

I did find that gray bar and attempted to drag to the left, but it wouldn't move.
The arrow on the right gives me the choice of "Questions" with my 2 slides listed.
It was there yesterday. I do wonder...I opened an old quiz from the last version of Articulate and a box popped up and said, "This quiz is from an earlier version of Quizmaker, would you like to convert it now? A backup will be created of the original file."
Thanks Erica for the image - if you've already tried dragging from the right to find the sidebar without success, I'd want to know next if a brand new .quiz file works as expected in terms of finding the form view. If not, you'll want to go through the repair steps here.

I am having the same problem but when I go to the right side and try to drag back to the left, nothing happens. Any suggestions?

Are you currently looking at the list of all the questions, like below:
If so, you'll need to click on one question to open it to see the form view vs. slide view.
If you're on an actual question slide and still having difficulty could you share a screenshot with us?
So, you can see that basically everything is showing except for that Form View. It came up automatically yesterday,
