High rate of "Incomplete" yet staff are scoring a 80 %, 90% or 100%, etc.

I am using Articulates LMS and I have a high rate of staff showing the Status of their education as being Incomplete. I watched a staff person complete the quiz, the results appeared on the screen as 100%, yet the status shows incomplete. She had to redo it and scored 80% the second time. This time the report I ran showed it as complete. She did nothing different other than select a wrong answer. They need 80% to pass. This is happening to a large number of our staff. Is there something I've done wrong in designing the quiz? Any ideas?
I am attaching the results for the same person that I've called Jane Doe. Very confused and looking for some help!
