How to add a text entry box in a survey qustion Quizmaker'09

Hello All,

Currently we provide the training feedback form as hard copy paper. I am looking ahead to make it online. Hence I am trying to set up a survey question in quizmaker'09 and add the questions. However, all the questions have 1,2,3,4,5 ratings and then an empty space for the audience to write additional information (optional). for example

What was your overall impression of your orientation? rating scale 1,2,3,4,5 then I provide a text entry field for the learner to write more. I have choosen this question as Short Answer type and provided the grading marks. However, I do not find an option to allow the audience to type additional information. Like a text box field for users to type or skip.

I am sure lot of users might be using this and I look forward for a easy and new idea.
