How to add Fill in the Blank box to Quizmaker slide?

I am still relatively new to Quizmaker.  I am editing someone else's slides.  A couple of fill-in-the-blank quizzes are missing the box to provide an answer.  Everything else on the slides is correct, it's just missing the box.  How do I add it back in??
