Hi Toni - What's the error message? I'm assuming you are utilizing the template here in this documentation. If you'd like us to take a look, please feel free to share the file.
A few things that need to be changed for it to work:
Questions 1, 2, 5, 6, and 13 are Multiple Response questions. The pipe symbol | is used to add feedback by choice in the spreadsheet. Since Multiple Response questions don't accept feedback by choice, you won't be able to add custom ones in the excel file.
The same applies to True or False questions (3, 4, 10, 11, 12, 14, and 15). On questions 3 and 4, you also have more than two answers (they should only be True or False).
