Inserting your name and company at the start of a quiz

Hi There,

I have done up a questionaire mulitple choice and at the begining I want the person doing the quiz to enter their name & company details and at the end I want to add a results page (not sure how to do this) to print off their results and also can the system email those results to me once the quiz is completed to avoid the need for them prinitng it off and scanning the results to me?.

Hi Julie and welcome to Heroes,

First, I'd recommend adding a "short answer" or "essay" survey question to the beginning of the quiz that asks for the user's name and company details.  On the pass result, you can select the option to "allow user to print results" and "allow user to email results to X".  For more information, see the link below:

Hi there,

I have now completed my questionaire and at the end you can print your results, I did a test of the questionaire and at the end I pressed the print results page and the screen came up but on the page there was no information of results only 2 colums displaying date & time, could you help with this please. Not sure what is going wrong.
