LMS (HealthStream) shows course in progress when quiz failed

My LMS is HealthStream. I have created a quiz in quizmaker. I allow the student to retry the quiz once after they fail the quiz. I have published the quiz using Scorm1.2 and report as passed/failed.
I have uploaded the quizmaker scorm file into the LMS. When I complete the quiz and fail it once, it allows me to retake it once. When I fail it the second time the results screen tells me I failed and I am not allowed any more attempts - as expected.
When I return to the LMS the course shows I am in-progress. I can click on the course (which only contains the quiz that was created using quizmaker). So, I can take and fail it two more times. Come back to the course, click on it and take it 2 more times....
To me it looks like my quiz is doing what it should be doing. But the LMS does change the course to "failed".
Here are a few things in HealthStream I have already addressed.
There is a completion mode drop down for the scorm (Completion Required/Not Required/Pass Required). I have tried setting this to Completion Required and also to Pass Required. The change does not effect the outcome. Also, when making the assignment I have Unchecked the box that states if a student fails to automatically re-enroll the student in the course.
I have already contacted HealthStream and they suggested checking with Articulate.
Any thoughts are greatly appreciated.

Hi, Diane -- So sorry for your troubles! Have you also had a chance to see how your course behaves when you test it in the SCORM Cloud, which is the industry standard for LMS testing? Here is an article on How to Troubleshoot Your LMS with SCORM Cloud that may prove useful.

Hi Christie, Thanks for your reply. I have sent you the file via your form attached. I have not uploaded to the SCORM cloud yet. However, as far as I can see it appears to be behaving as I expect it to except for the fact that HealthStream is not completing the course. I'm not sure what they are expecting to see to complete the course and I could not get that information from their help line.

I did not get an acceptable resolution. I ended up re-creating the quiz inside HealthStream. It looks like the issue is that Quizmaker keeps the quiz set to "in-process" and never actually records a "failure" back to HealthStream. - I had to go back to my email thread to remember the details to this issue.
