Passing Score, Failed Quiz in SCORM Presenter lesson

One of our students ran into an unusual issue recently. They passed the quiz in a lesson, but Quizmaker stated that they had failed. See the attached image for an example:
We've had about 1200 or so students complete this lesson previously without this problem occurring, so I suspect it might be related to a file getting corrupted in transit, a file being corrupted on our LMS, etc.. However, I was hoping you might have some ideas on what might cause a problem like this one. I've seen in other posts where similar things would happen if you don't have "Complete/Incomplete" marked in the SCORM settings, Don't allow multiple attempts, but we did. Besides, I'm not sure why this problem would pop up now.
Any thoughts???

We have had this happen once, if the user had not sent me a screenshot I would have thought they were crazy. They took it again and passed. We put it down as a bug that must need really rare criteria to take place. Over 5,000 completed courses only seen it once rare but it happens
