Quiz sending updated as Passed when it is incomplete

Am using Studio 13. All of the lessons we produce in Presenter include an end of unit quiz set to pass at 80%. All completion is set to be triggered by the quizmaker quiz. We publish to an LMS using the AICC setting.
From time to time for no apparent cause we will encounter a situation where the lesson gets marked as complete when the student hits the first screen in the quiz before they ever complete the quiz (so no score, not just failing score).
When this happens our only recourse is to publish again and again and again. We will make random minor edits to make sure that the quiz is saved with an updated date/time and then reinsert it. Eventually through trial and error we will get it to publish without this bug.
We have not been able to find any articles related to this after searching several times. Has anyone out there had this kind of problem?
