Have you looked at testing this within SCORM Cloud? It's an industry standard for testing LMS content so you may want to try uploading there. Since you shared your published output here, it's difficult to say what properties or items you selected in your publish settings but if you have your .quiz file I'm happy to take a look at that.

I removed the post. But just so you know, the little picture of the woman with gray hair looks a LOT like me. the only difference is I have asymmetrical hair (above the ear on one side.)

You'll also want to ensure you're viewing the course within a supported browser.
