Reports show that quiz is incomplete and no grade

Quiz has a blank slide to start with a movie. Then 3 questions to answer for a total of 4 slides. The movie slide is not part of the question count. When users watch the movie then click next, they anser the questions and close and logout. My reports show the following
Shows incomplet and no score. I can't seem to fiqure this one out. any thoughts???
