I created a 6 question evaluation survey in quizmaker '09 and dropped it into a presenter '09 shell. I am able to publish and zip the file. I have uploaded it to our test LMS site as a resource and attached it to an instructor-led class for learners to complete after the class ends. I cannot get it to open and play from the LMS.
The zipped file icon is different - like it is missing a file.
If you've followed those steps, can you tell us which LMS you're using?
Also, have you tested your course in SCORM Cloud? You could also try loading your project in Tempshare as well, just to see if it's an issue with the LMS itself.

Let me know if you see the same thing on a server outside of the LMS. If not, I would recommend reaching out to your LMS to see if they require a different upload process or if it's a common issue they've seen with courses.
I reviewed the article referenced above and everything seems to be correct. When I uploaded the survey to the SCORM cloud it worked correctly. However, when I try to upload it to the template I get an error "Sorry we are unable to publish this file."
If it's working in SCORM Cloud, sounds like it may be something with your LMS as Christine shared above. Hopefully they will be able to identify what may be going on from that end.

Sorry to hear about the trouble. Hopefully we'll be able to figure out why the presentation isn't loading properly in the LMS.
