The built in question types for Storyline only allow for one question/interaction per slide so you'd need to set this up in a more customized way using the button sets and text entry on a slide. There are a lot of examples on how to set up multiple of the same question (fill in the blank, multiple response, etc.) and hopefully these will help get you started.
