Adding TextEntry field to a Survey Question

Hello,

I have a module made up of survey questions using "pick one" or "pick many" templates. I want to include one survey response as "other" and then add a textentry field so that the learner can type in the additional information related to "other." When I publish and post to moodle, the report shows "other" marked but I can't seem to get the textentry field to report what was entered there. 

Is that possible to do? I appreciate any thoughts someone might have on this.
