Adjusting Variables with Button Sets

Hello,
I seem to be having difficulty identifying my error in adjusting variables for the attached course.
As you can see, (scene 5) I have attempted to create a Likert Scale with Radio Buttons in order to track users who have selected "Not Beneficial" or "N/A" for their choices. The main goal would be to not allow the user to reach full completion mode in the course, upon publication, if these options are selected. I have attempted to redirect the user to a different concluding screen (scene 6) which would instruct them to contact their manager for additional instruction. As it stands, I have tried setting up an Advance to the Next Slide Trigger if the selected Radio buttons variables have been adjusted to True, with no luck.
Button Organization:
4 Questions
4 Button Sets of 3 (Identified as Q1,1 for Question 1, Radio Button 1, Q2,1 for Question 2, Radio Button 2, and so forth)
Any help on identifying the main issue would be greatly appreciated.

I can't quite follow the logic, but the issue may be with the submit button trigger that jumps to slide 5.2. In the conditions for this trigger, you 'mix' OR and AND statements which doesn't work. You need to split these up into several triggers.

I suppose the logic behind it is that I want to track users on their scaled response, which isn't possible in the Survey Question (or so I thought?) Maybe I'm going a round-about way! But thanks for pointing out the mixing of variables!
