Best Approach to Identifying Parts of a Form

I am creating a Storyline 2 WBT on the attached UB-04 form. Each field on the form has to be explained. I'm not sure the best way to go about this. Do I use markers? Can shaded hotspots be used with pop-up information? Is there a way to embedded the whole PDF file and magnify it on one screen? Or split several sections of the form across different slides? How would you explain the content vs how to assess learner knowledge of it (part of the way through and/or at the end)? Just looking for the best way to explain/assess this while also not spending an inordinate amount of time creating it (since there are so many fields on the form). What are your thoughts? Thanks!
