Best method to have an elearning produce an assessment that is captured in an LMS?

Hello e-Learning heroes community!  I have been struggling with my modules I am builder for a client.  The desired result I am going for is to have several text entry fields throughout the course and ultimately have those responses pass onto ONE comprehensive report that captures the initial question, there answers, and is carried over to an LMS.  My initial attempt was somewhat successful in that the entry fields go onto a new slide where users can print or email their response per each assessment.  It required a lot of trial and error using script (which I am not an expert at).  My thought was to combined all the script into one and see if all the entry fields can populate on one slide though I have reservations about getting all the text onto one slide layout.  The next obstacle would be to assure that Articulate will allow me to publish with this report being transferred to the LMS, and to have the report or assessment be presented in a nice format that is captured in the LMS but can be printed out for a user to keep for records.  DOes anyone have an expertise or insight on to how I can achieve this?  I can share my existing project if anyone is interested in viewing.  Thank you.
