Best Way to Export User's Text Entry

Hello!

My end users need to fill out a declaration form within a training, and we need to be able to collate all of the entries into a spreadsheet.

For example:  capture text entry of name/employee identification; capture which questions they responded YES to, and any text entry.

Julie F. #12345. 

Q: Do you have a pet?  - YES

Q: What kind of pet?  - Tortoise

Harry B. #34567

Q: Do you have a pet?  - NO

Alex Q. #09876

Q: Do you have a pet?  - YES

Q: What kind of pet - Hamster

And then I would need to collate this into a file that I can send to Excel via a database command (?).  Google Sheets is not an option for me, due to security issues. 

To further confound the issue, I have multiple platforms with this need. One does not have an LMS and will host it on their web. Others have various LMS solutions... one is a super old version of SuccessFactors and only supports AICC content...

I imagined that one could do this with Text Entry variables and some sort of Javascript to create a .txt or .csv data to be collected, but I am not seeing anything that really addresses this. 

Or is there another solution, such as creating a fillable PDF, inserting it into the training, and extracting the data from that?

I think this can be done if your hire some developer. Because my project is some thing similar about pets so after lot of research i have to hire developer to do this job.
