Building a post-program survey

Hi, everyone.  I work for a health care organization where clinicians and nurse practitioners do their residencies.  The residencies can last for several weeks to half a year.  We want to do a series of surveys to ascertain the effectiveness of the various programs.  We are looking at a combination Likert and open-ended questions.  Examples for Likert are, "My clinical mentor was able to balance their clinical duties while providing me with the foundational knowledge I need to move forward."  The open-ended questions would be, "What are some actions we could implement to improve this program?"

I imagine many of you have done this in the past.  I'm looking for templates, and suggestions for how to make this as effective as possible.
