When you add pre-made button from the insert tab, the states should appear automatically. However, depending on the button you choose, some will not have the "visited" state, e.g the Check mark buttons do not have this state, but the Disabled should be there.
Do you have a file we can look at, or a screenshot of the button you are trying to add?
