Can this be done using Articulate?

We have an audit training form for our new auditor on-boarding process and it basically requires the auditor to fill in the blanks with completed training and the trainer's initials. My manager wants this to be placed in an "e-learning" format and I was referred to this software by a co-worker. Is there anything that I can do in Articulate that would fit the needs of this particular situation. I am not sure if the software has a category for this.

Hi Lynette: Yes and no. It will depend on what information you want to keep track of, what feeds back into your LMS, and how you can report on it. (if you are putting it into an LMS).
Ours, unfortunately, only does pass/fail. It doesn't track to a granular lever how a question was answered.
You could set up each of your "check list" items as a question and maybe track answers that way.
I just recently set up several "online checklists" as courses. Basically, each slide listed several tasks that need to be completed to demonstrate compentency in a skill. The learn says Yes I know how to do these or No I do not. If they say no to any one of the Skills, the course stays active and by using variables on each slide, the final slide checks of the Skills they indicated Yes to and the Skills they indicated No on. Saying yes to all is the only way to pass.
It puts a lot of trust and honesty on to the learner, but the current method was no different as they checked yes/no off on a piece of paper and signed it. I essentially just recreted it in a paper version. So in my example I am more so just capturing "you click here and you are acknowledging you know this". I'm not tracking dates, other than the date they click YES.

Hi Tracy - Any thoughts about using your approach to fill out something like this? I have tried Multiple Response (only has 10 answers) and Pick Many Freeform (only allows one answer) and have come to a stand still.

Can you expand on the context of how/why you want them to fill this form out?
I have done something similar for an exercise within a course and as a quizzing item. If you can give me some context, I can better understand how to share what I created.
It is an elearning on how to fill out the form correctly to get to the correct action. It has some complications in it (below) but we just want them to know the process. It is the first of a few scenario based questions to practice real-life situations.
I do have a solution, but it's a bit complicated to write out here. I'll email you directly.
