I'm looking for a simple way to capture and report to the LMS the name, the date and a typed comment (''I agree'') for each learner who completes a course. The learner can type their name, the date and type ''I agree'' to accept the terms displayed on the page. (Or the ''I agree'' could be a button.)
This info would then be captured in the LMS
I'm not sure where to start with this - any ideas?
You could have a button that says "I agree" the terms of this course (blah blah blah legal requirements/disclaimer etc.) and have the button trigger be to close the course.
Every LMS that I have seen would report the completion of the course including the person, and the date/time.
If you want to do it with a radio button, then the agreement would be test question with a single question. If they agree they pass the course. Your LMS should have the ability to run a report to show who has passed that course (agreed to the terms).
I thought about it. But was wondering if it could be done with a radio button. Also I liked how you tied the 'exit' function to it. I was tracking with slides viewed. BUt your suggestion is more accurate.

Neat, I will try it. Thanks so much.
I am looking to make it like a radio button. Can I make it with a trigger too?
Thanks again.
Best
One quick question, how can I measure within the LMS system that users had completed this course after signing? thank you
Sorry forgot the attachment.
