As you know actions are carried out when a trigger is fired so in your case you want something to change state of an object if I understand you. This will happen when a certain time runs out.
My solution would be to create a thing and place it on your timeline, but offstage.
Shorten its life on the timeline to say 4 seconds then use that to trigger the change state of your object.

Bruce is absolutely right. The easiest way to achieve what you wrote is to create an object outside the slide, then add a trigger to change the state of the caption when the timeline of the 'outside slide object' starts. You can place this element wherever you want on the timeline.
If you don't mind me asking, why would you change to state to visited after a certain time? The visited state is usually used when you want a learner to see what they've clicked on.
