Changing cursor to a speaker when hovering over a hotspot

Sorry... but cannot seem to figure this out.

I want the cursor to change to a little loudpseaker when the cursor is hovered over a word. Any ideas for doing this? I also cannot seem to find such a cursor in the collection of cursors. Can I add one and, if so, how?
