Completing a form online

I have articulate 360 and I am thinking of using storyline to produce an online revalidation process for our staff. This will require them to complete a mix of tick box and open questions. It would be nice to then have the information print as a completed form with clear headings and formatting. Before I go jumping in feet first could I please ask anyone out there is this possible and if not is there a better medium you could suggest?

Our company uses SmartSheet for things like this. It's not a huge learning curve, the data is collected in an Excel-like database sheet, so it's easy to manage and design, and the forms you create to feed that sheet are modern and clean and intuitive. I'd recommend it.
You could download as a formatted pdf will need a bit of coding however it is relatively easy to achieve.

Yes, you can generate a PDF file with the user entries. They can download/save/print the file as needed.
