Complex form interaction

Hi there. I'm a new Articulate Storyline user and have a question about creating a practice-type question in the tool.
I would like to show an accounting form that includes several fields. Some of the fields are filled out correctly and some are not. The learner must:
1. Determine which fields are incorrect
2. For incorrect fields, determine what the correct text should be (from a list of possible choices)
Is there any way to do this in the tool? Or other ideas on possible "work-arounds" for this idea?
I've done some research, but so far everything I've seen limits you to one free-form enter per screen and I need to include at least 3-4 per screen.

I hope someone in the community can share with you an example, but in the meantime I did want to point you to this tutorial workaround on how to evaluate multiple fill in the blank questions on one slide and this additional thread with numeric text entry.
