I have just launched an Annual Compliance course today and have run some reporting which shows the course is marking itself complete even when it the user has failed as well as no score.
I've compared this to my many other courses and cannot see anything different in terms of the tracking and settings but I'm hoping there is something I can fix rather quickly as its already live.
They can complete the course and still fail but if you have selected to report the score it should be doing that if you have selected Passed/Incomplete or Passed/Failed.
Try checking what happens in Scormcloud (it's an industry LMS that is good for testing) and see how the course reports there. If it reports correctly then it may just be an option to tweak in your LMS.
Maybe let peeps know which LMS you are using and they can chime in if they have a solution.
I did have the course tested in Scormcloud by the wonderful Hazel on Articulate web chat and it did send a score.
My LMS provider, Skillsoft Percipio, advised their platform follows the SCORM specifications to the letter and actually requires a message of passed/failed which is what I've included but they cannot seem to see why its occurring from their end which is odd because I've created many courses without these issues.
I'm just making sure I haven't put too many triggers on the results slide or if they are in the wrong order which may be causing an issue, if that's possible at all.

I will try this now, do you think it would also be worth trying to add a completion trigger as well? Or am I now adding too many things
It could be to do with how your LMS reports Liz.
