Creating a user

I am using Articulate Storyline to create a Health and Safety Induction Training for new employees. I would like to know if it is possible to create a page/slide at the beginning where the employee can insert; their name and the date they are conducting the training. I would like to then use this information in the final results slide, so when they go to print it, this information is already entered and can feature on the printed 'certificate' which we would like to keep on their file. Thanks
