We want to put several company policies online and have an acknowledgement page at the end for employees to click on as an electronic signature. I am a beginner and would appreciate help in how to create that page. Thank you!
Just an FYI - responding via email includes you're signature here and you're welcome to edit the post to remove that information if you wish. I was thinking of doing this very interaction, however, I need this to report to an LMS.
Do you have experience using something like this on a Quiz that will report PASS(acknowledge) or FAIL (not acknowledged) to the LMS?

You could turn this into a Freeform Pick One question, with the checkbox being the correct answer. Your 'quiz' result could then bve reported to your LMS as a 100% passed.
