Custom quiz or embed web object help

Hi everyone,
i need to create a quiz/survey where the learner can answer yes or no to a question and if they select no, can then type in comments as to why. There are to be 6 questions in all.
I could not see a way of doing this in Storyline? Is it possible?
as a workaround, I have created a quiz in Smart Survey and it provides me with a URL to the quiz. However if I try and embed this via web object in Storyline it fails to work when the file is published, but does appear when I click on in slid preview.
Can anyone provide help or way of doing a custom quiz within Storyline to achieve the desired result?

Hi Jeff,
I answered a similar question the other day, with an example included here. Would that work for you?
I've not heard of Smart Survey, but I know there are community members who use Survey Monkey, especially if not using an LMS to track results.
