I am an ICT teacher at a school, and have created a solution to save all results to a database automatically after the quiz. I am using a MS SQL Server database, but it could easily be adapted to a MS Access or other database.
We have already taken hundreds of tests, and all results are stored, up to individual answers. The web interface enables you to download results per quiz, per student or per class in CSV format.

If you're looking to find other users to collaborate with, you may want to post the information as a new thread so that those users who are interested can organize in one central thread.

I do wish other people to help me with the project though.
I'll try a new thread but perhaps a better way is to use a personal message to people with the same problem. I think a general post will be too easily overlooked.

It probably depends on what type of form you want to make. Are you planning to use just data entry text fields? If so, you could report the variables to an LMS.
I'm not sure if that's an option for you, but it might be worth checking out :)

No worries Michiel - we saw it was open source, but after a few got by as spam. Sorry about that.
We don't have private messaging here in the ELH forums, so that's why I thought a new thread may be more focused than blanketing a few threads.
