I imported the "index card" quiz template... and now any time I go to add a quiz slide it automatically put that template on every slide... I just want a blank template...
Sounds like your new quiz slide is picking up it's style from a slide master. You may be able to change the slides layout to a blank or generic design.
When you import a template the styles for that template are added to your projects slide master as available layouts for new slides.
You should be able to edit the layout for any of the slides in your project to match the style for any slide master, if that makes sense. Very similar to the same functionality in PPT, if you've ever used slide masters in PPT.
