Discussion threads for each screen

Has anyone here ever implemented a solution by which each screen within your course has its own discussion thread? I took a Udacity course that taught the content via a YouTube video and then each different video had a discussion thread. This allowed learners to ask very specific questions about the content contained within that smaller (2-3 minute) video. 

I know some LMSs offer discussion thread capabilities, but I would like to offer discussions within the course at the screen level.

I was thinking there could be a notification on each screen whether there had already been posts related to that screen's content.
