I have an issue with a drag and drop free form quiz. I have checked "Allow only one item in each drop target", but the published version intermittently allows multiple items per target. As a result an item is unexpectedly hidden behind another item.

Hm, that doesn't sound right. I can help investigate, and have few questions to start:
What version of Storyline are you using? You can find this detail by clicking on the Help tab, then About Storyline.
Where is the published output hosted?
Would you mind sharing the .story file (even just the drag-and-drop slide is perfect) so I can try to replicate the behavior? You can share it publicly here, or send it to me privately by using this upload link. I'll delete it once we solve this puzzle!
