Hi, Jerry -- Thanks for reaching out here! While I will defer to your fellow community members to share their ideas and advice, I thought I might pass along the following in case you might want to take a look:
Where possible I would recommend using your LMS's survey functionality instead of Storyline. You'll generally get much better reporting capabilities that way, and you can standardize the survey questions to compare across courses as well.
If you're just publishing to web or CD, I'd recommend embedding a link to an external survey (e.g. surveymonkey).
