Email/form to report issues or questions on a course

I thought I saw this discussed before but just can't find it.
I would like to have a form/email where users can report an issue or ask a question about a course. The issue is--most of my users don't have Outlook installed on their computers for using the email link (and I can't have it fill in course specific info so I know the course and slide they are on). So, I was looking at just setting up a surveymonkey link where I require them to fill in the info to do as a lightbox slide for this, but realized I can't have surveymonkey automatically email me when someone responds which would be pretty important! Ideas? Suggestions? Have the link to the discussion?

Have you tried Google Docs? You can set up forms that store input in a Google spreadsheet, and that will also notify you when a new entry has been added. Google forms can be embedded in HTML, so you should be able to add it to a web object.

Story
smart survey will email you each time you get a response. Also you should be able to insert it as a web object
