Embedding link to external survey causing incomplete status

Hi everyone:
I am having an issue with a module that was functioning fine but needed an update with a link to an external survey. The survey is mandatory (but is only 2 questions) so I set it up so the module could not advance until the link was opened (the button was hidden until the link was clicked). We are having no trouble getting the link to open, and the learner can navigate to the end of the module easily. However, since adding the link and testing in SCORM Cloud, the course will now fail to complete. There are three choices of types of links and we've tried all three and none allow the course to complete. I took a screen shot of the three links just in case that would be helpful.
Any ideas?

How have you set up the tracking within your course?
Are you adding this survey as a hyperlink to a new window or inside Storyline as a web object? It may help to take a look at this .story file to see how everything is behaving - can you share the .story file here with us?
I set up tracking so that 21 out of 26 slides need to be visited. I can't attach the file - I've tried and keep getting the 404 error message. I'll try copying some of the slides into another file and set similar tracking. And I guess if it works then it will just muddy the waters even more.

The file may be too large to share here in the forums (around a 20 mb limit) or your server may be blocking you from attaching files. I can send you directions on how to send to me privately if you're unable to post here.
Here's the file with a few of the slides including the problem second last slide. I will be interested to see what you come up with.

It’s not that I didn’t want to post it, I just kept getting the errormessage so I copied the last few slides and set the tracking similarly andposted it.
If this doesn’t work and I need to send the whole module, I’ll followyour instructions to send it privately.

Oops. Apparently you can't just cut and paste from Outlook without getting all that other stuff before the message. Oh well!

Thanks for sending it along. I tested it out at SCORM Cloud which is an industry standard for testing SCORM content and everything worked as expected. I entered in some test survey answers, close that window was returned to the the slide where I launched the survey and then proceeded to the final slide where I exited the course. I took a look at my status in SCORM Cloud and it showed me as passing and completed as I met your tracking requirements. If you'd like to test the course as well you can access it from this link, and when you close the course you'll be presented with a window which includes your results like mine.
Thanks Ashley. This is one of those puzzlers because we did test it in SCORM Cloud and it didn't pass. Maybe there's something we're doing with the survey that's different. Or maybe the copying over to another file removed the slides from a problem existing in the course. Well, at least we know that it is possible to pass it, and maybe that will help narrow the issue down.
Thanks Dorothy - feel free to let my link as well and let me know if you receive other results. If there was a problem within the .story file itself, you may want to try importing your existing file into a new Storyline file as described here as that can help resolve issues.

Thanks for sharing your project files. I took a look at your original file, and published it to SCORM Cloud - I went through all the slides, launched the survey link (although I didn't take it cause I already had and hadn't cleared my cookies) and returned to finish the course. Again I saw a completion status of complete - but my success status showed failed. When I looked at your publish settings, I saw you were using Complete/Incomplete which won't report a a success status, so I changed the setting to "Passed/Incomplete" and then reloaded the course to SCORM Cloud here: https://cloud.scorm.com/sc/InvitationConfirmEmail?publicInvitationId=d5568e0b-1a1e-481f-9eda-5f0fee1c8e38
On this test, I saw complete and passed - so it seems everything in this version works in SCORM Cloud with those settings.
