Embedding MS Office Form (to rate a module) in Rise

I've created an online review form that displays well (desktop/android) when embedded in Rise (before the learner exits the module). But it does not work well on iDevices. The MS Forms image displays, and not the actual questions; the learner has to click to 'fill the form'; leaves the module to complete the review and after submitting the review; has to navigate 'back' to the online module to exit/complete the module. We could make it work (with many learner instructions, etc.). But, we were hoping for suggestions to improve the iDevice display(similar to desktop/android)?
