Error When Inserting Question Bank

I recently did Update 4 for Storyline. Since that update, I am experiencing an issue when I attempt to add a Question Bank to the course. I imported a Power Point (v. 2003) presentation for a background template the client wanted to use, but built the slides in Storyline. At the end there is an assessment. I created a Question Bank for the assessment, and when I go to insert the Question Bank, I get an error message asking me to write details and "send report" to Articulate. Has anyone else had this issue that may have a solution?
