I need to create a survey in which we want a clear Yes/No answer as well as a text entry field to provide users to further elaborate. I tried what I knew but could not get it to work. Any ideas?
I'm sure others will have some more elegant solutions, and there are a few threads on multiple questions/text fields on slides (like this one and this one), but I thought I'd chime in on the topic.
What I would do is create a "Pick One" question with a text entry field. That text entry field will create a variable. You could then report that variable to your LMS, in order to review the text your learner has entered.
Again, that may not be the quickest solution, but I thought it might help - especially if you're only intending to do this with one or two questions, versus many.
