Fill-in-the-blank type question

I was hoping to make a single quiz slide with several "fill in the blank" sentences for the user to input their answers. (E.g. An example of this is _______. ________ is the most common form. Another example is ______.)
I have tried using shapes and text boxes & converting them to freeform text entry fields but it doesn't recognize either of those things as possible text entry fields for the quiz (it makes a new text entry box), and only allows one per slide.
I've contacted support and such a feature is not currently supported. I was wondering though if anyone has been able to achieve a similar type of slide in another way? I would prefer not to have several different slides each with only one sentence.
