Sorry, it was Storyline 3. I see the problem now, but I can't seem to replicate it in a new project. Was this a new project you created or one that you upgraded from Storyline 2? I'm running Storyline 3.0.10762.0
I created a new project and did not have the issue, but did have the issue with your project. I'm not sure what is causing the problem, but hopefully support will get a chance to look at it today.

Gary's issues seems to be different than mine. If I publish flash only it works fine on LMS and offline locally. In HTML 5 when i get to question bank 8 in review, the course goes blank, in lms and offline locally the review doesnt launch.
I've attached the storyline file for the quiz portion of the course.
I gave your file a quick test on SCORM Cloud, which is industry standard for LMS testing. I like to compare course behavior in SCORM Cloud to the behavior in another LMS to parse out file issues, and your file appears to be working normally there!
Would you mind testing this link and letting me know how it's working for you?

After completing the quiz, clicking review, it takes a long time to load
then starts on question 9. When clicking next when i get to question 8 the
screen goes blank.
I received a response from Vevette Chua below, saying that she was able to
reproduce the issue and would investigate further.

Are you able to post your project file here? Does it work as expected outside of your LMS?

I made a simple file for testing.
Please help to solve our problem.
Gary, what problem are you having? Your project seems to work okay for me.
If I publish to Html5/Flash or Html5 output format, the review button doesn't work.
I use Storyline 360 to publish.
Michael, What version of Storyline do you use?
I think I opened your project in Storyline 2. I'll go check that review button now.
