Are you referring to hotspots in general, or the hotspot quiz interaction?
If you are referring to the quiz interaction, you are restricted to what the interaction has set up. I'm not familiar with it, so I'm not real sure, but I think you can only have the one correct answer.
You can place hotspots anywhere on you presentation screens that you want, and set triggers on each of them to perform however you want. If you want to use a general slide to report as a quiz interaction, you can set up your interaction in the general slide, and then trigger a question slide with the appropriate result. This will allow you to have the flexibility to program your presentation to perform the way you desire, and still report the results to the LMS. I've used this technique many times to create a custom interaction that goes beyond what the default quiz screens are capable of.
