We are currently creating a survey and would like the questions featured to show up on the Report. As of now, we have added the question on the slide title and in the notes and we cannot seem to have the specific questions pull up in the reports. I have attached a few screenshots to demonstrate.
I do have a results slide and 'Submit results 15.6 Results Slide' on the trigger section. I have attached how the slide questions show up in the Edit Results Slide.
By the looks of those screenshots, it appears that you've set everything up correctly. From here, I'd like our team to take a peek into your file to see if they can spot anything that would cause this reporting problem.

Does your course contain a results slide? And does that results slide have a "Submit Results" trigger in the trigger panel?
